  • Summary of Material Modifications (SMMs), which are separate documents that describe material changes made to the plan (i.e., changes to the plan that are not yet reflected in the current SPD)

The Health & Welfare Administration section of this website covers information that applies to the health and welfare benefit plans. These documents are separated from the specific plan details to minimize repetition and to keep related information together. These documents together with the above listed summaries are intended to be part of and complete the summary plan description (SPD) and/or plan document for a plan. Note: This Administration section does not apply to the Retirement Savings Plans and Retiree Benefits Program, as the applicable content is already embedded in their respective benefit plan documents. Documents in this section include:

  • Plan Administration, which provides administrative details such as plan numbers and statements of your rights, including your right to appeal, which is required by law
  • What Happens If …, which describes how different life events and situations can affect your benefits and/or provide you with opportunities to adjust your benefits coverage
